Red blood cell mass


Red blood cell mass is the main hemotransfusion medium, with a hematocrit not exceeding 80%. Red blood cell mass is obtained from stored blood by removing plasma. Transfusing red blood cell mass is the preferred method for restoring the oxygen transport function of blood. Compared to whole blood, red blood cell mass contains the same number of red blood cells in a smaller volume, but significantly less citrate, cellular breakdown products, cellular and protein antigens, and antibodies. Patients with chronic anemia, heart failure, and elderly individuals poorly tolerate a sudden increase in blood volume; therefore, transfusing red blood cell mass is most appropriate for them when oxygen capacity of the blood is reduced, as minimal increases in blood volume due to increased circulating red blood cells significantly improve oxygen delivery to tissues. In addition, non-hemolytic transfusion reactions occur much less frequently during red blood cell mass transfusion than during whole blood transfusion.
