Statement for coordination of the establishment of control by a foreign investor or a group of persons over the specified joint-stock company - addressed to the authorized body and composed in an arbitrary form, specifying the type of activity and the list of works, services that are performed, provided as part of that type of activity, for which obtaining a license or making changes to the license register or obtaining or reissuing an accreditation certificate or a conformity document is required¹.
