Sports Passport
1. The sports passport is a standardized document certifying affiliation with a physical culture-sports or other organization and the athlete's sports qualification.
2. The sports passport contains the following information:
1) the surname, first name, and patronymic of the athlete;
2) gender;
3) date of birth;
4) affiliation with a physical culture-sports or other organization;
5) selected sports and disciplines;
6) information on the assignment of sports ranks and titles;
7) information confirming compliance with norms and requirements necessary for the assignment of
8) notation on the athlete's completion of medical examinations;
9) results achieved at sports competitions;
10) information regarding sports disqualification;
11) information about state awards and other forms of recognition;
12) surname, name, and patronymic of the coach;
12.1) information about organizations where the athlete underwent sports training and the duration of such training;

13) other information related to the specifics of the chosen sports disciplines
