A request to change the surname, first name, patronymic of the child being adopted, the date of birth of the child being adopted, the place of birth of the child being adopted, and to record the adopters in the act of birth registration as parents — at the discretion of the adopters (adopter) to make the corresponding changes in the birth registration record1.
