TransTek
TransTec - a recently registered in Russia transdermal form of buprenorphine, created using a special technology and representing a skin patch with buprenorphine incorporated into a special matrix base. Buprenorphine, like fentanyl, possesses the necessary physicochemical properties for transdermal administration, in particular, high lipophilicity. Upon application to the skin, buprenorphine gradually diffuses from the matrix through all layers of the skin, entering the capillary and then systemic circulation, exerting its analgesic effect. The drug is available in three dosages: 35, 52.5, and 70 mcg/h, corresponding to amounts of buprenorphine of 0.8, 1.2, and 1.6 mg over 24 hours. The analgesic effect of the applied patch lasts for 72 hours. TransTec is effective for moderate and severe pain of any origin. It is used for the treatment of chronic pain not only of oncological but also non-oncological origin due to its long duration of action, convenience of therapy, and low risk of dependence. Clinical efficacy, like other formulations of buprenorphine, may be limited by the "ceiling effect" of analgesia, which may require switching to morphine sulfate or transdermal fentanyl therapy. TransTec is not intended for acute and episodic pain due to the gradual (within 1 day) onset of analgesia and prolonged duration of action. Therapy with TransTec is generally well tolerated by patients, although opioid-related side effects may occur (sedation, dizziness, nausea, occasionally vomiting); constipation with TransTec use is very rare.
